1.Determine how much money you need to have financed.

• Medicard can finance the entire procedure or a portion. You decide.
• Medicard can pre-approve an approximate amount. This allows you the flexibility to choose your physician and procedure date with the comfort of knowing that financing has been confirmed. Upon finalizing these decisions, steps 6-9 will follow.

2. Finalize the date for your procedure.

3. Choose your ideal loan terms: 4 or 6 months; 1,2,3,4,5 years.

4. In confidence, submit your completed application securely online or by fax.

5. Medicard will notify you of your approval by phone.

6. Medicard will confidentially fax the loan document to you for signature.

7. You sign and fax the loan document with a void cheque to Medicard.

8. Medicard pays your doctor directly.

9. Your monthly payment is electronically debited from your chequing account.
